Established Patient Forms

If you are considering Artificial Insemination (AI) or Intrauterine Insemination (IUI) please complete the AI Participation Form. If you are considering an Assisted Reproductive Technology procedure, such as IVF, please complete the ART Participation Form. Please turn in completed forms at least one month before the expected cycle start date so that our billing department can determine insurance coverage and obtain pre-authorization for treatment.
